The Advanced Pharmacy Practice Experience (APPE) Transition of Care Rotation (TOC) rotation provides practical experience for clinical pharmacy interns. The experience they get familiarizes them with the role of the clinical pharmacist in the hospital - inpatient setting. They also obtain expertise in optimizing pharmacotherapeutic management. Further, APPE enables interns to gain knowledge of improving patient care and safety.
